    The Chronic Disease NP will have a significant nursing history related to complex multi-comorbidities, and years of experience as a NP working with high needs patients with Health Disease, Diabetes and Lung Health problems.

    Using evidence-based approaches, the NP will collaborate with the patient's GP, the practice clinical pharmacist, the nursing team, the Health Coaches and Health Improvement practitioners to ensure a high standard of care and health outcomes is achieved across the practice.

    Requirements:
    • New Zealand right to work, and NP practicing certificate in New Zealand.
    • Significant experience managing patients with Cardiac Risk Factors, and Diabetes
    Looked upon favourably but not required:
    • Experience working in Primary care setting
    • Experience working with Lung Health chronic conditions
    • Excellent references from doctors with a 5 years of direct working relationship experience.
    • Experience working collaboratively on patient management with a multidisciplinary team.
    • Certification as a diabetes educator
    • Experience working with patients directly on lifestyle change interventions, (such as smoking cessation, diet improvement, fitness activation, mental health practices)
    • Experience with Te Tiriti o Waitangi in delivering health services.
    Position details:
    • .6 FTE - 3 days of (Tues, Wed, Thurs, Fri)
    • 3 locations of work, all located in Silverdale/Millwater on the Hibiscus Coast
    • No parking at clinic site (free parking is available 5-10-minute walk from clinic sites)
